Applications for the testing of vehicles intended for the carriage of dangerous goods by road should be made directly to DVTA. The address is shown below. Application forms are available at all test centres and here at the DVTA website
Application form
Testing is only carried out at Belfast and Cookstown centres at present. Where possible, all applications for the Carriage Of Dangerous Goods test should be made at the same time as and the same centre as the annual test. If this does not occur the period validity of the CDG/ADR certificate may be reduced (see FAQs page).
Forms must be fully completed and be clearly legible. Incomplete or illegible forms will be returned to the applicant. When completing the form, applicants should seek the advice of their company safety advisor in relation to any matter that they are unsure of.
They must ensure that all documents required for the issue of a certificate of approval are submitted with the application (copies only are to be sent, but originals must be presented to the examiner on the day of the examination).

Where a vehicle fails a CDG/ADR examination the applicant has 21 days to apply for a retest, or the full fee will be payable.

The vehicle examiner may not undertake an examination unless a cleaning/purging declaration has been properly completed. These are attached to the application form, however they are also available at all centres and on the DVTA website and must be produced when asked for by the vehicle examiner at the time of testing.
Other Declarations listed in the application form depend on the CDG/ADR type of vehicle and, where appropriate, details of the substances to be carried must accompany the application. For tank vehicles requiring a CDG/ADR certificate, a tank type approval certificate and a tank initial inspection approval certificate will be required and copies must accompany the application. In addition, where relevant, a periodic tank inspection test certificate and/or a tank leak proof test certificate will be required and again copies must accompany the application. Copies of tank certificates are to be submitted at the time of application with the originals being produced on the day of the examination.
