GUIDANCE NOTES FOR COMPLETION
PENALTY: The penalty for knowingly making a false statement or withholding material information for the purpose of securing registration is level 4 on the standard scale, currently £2,500.

PROOF OF RESIDENCY
The transfer of an ADI licence from Great Britain to Northern Ireland can only be processed if the applicant is resident in Northern Ireland and proof of residency is provided to DVA. Acceptable residency documents are:
- gas, electric or telephone bill
- credit card /bank statement
- agreement to sign up for a utility such as gas, electric or telephone or a tenancy/mortgage agreement
CRIMINAL HISTORY CHECKS
Each applicant applying for a transfer of registration will be subject to a criminal history check. These checks will be carried out by Access Northern Ireland (ANI). ANI provides criminal history information about anyone seeking paid or unpaid work in certain defined areas, such as working with children or vulnerable adults. ADIs will be subject to an enhanced check under ANI. In certain cases the disclosure may contain information about charges or cases pending which have not yet resulted, or did not result in convictions but which the police believe may be relevant to the position of an ADI. ADI Section will provide advice if requested to new applicants with concerns about a criminal history. DVA will make decisions on the basis of its 'fit and proper' person policy which can be provided to applicants on request. Enhanced disclosures obtained for the purpose of registering as an ADI in GB cannot be accepted.
IF YOU DECIDE TO APPLY ONLINE DVA WILL FORWARD YOU AN ACCESS NI APPLICATION FORM FOR YOU TO COMPLETE. IT SHOULD BE RETURNED TO BUSINESS SUPPORT UNIT, ADI SECTION, DVA, BALMORAL ROAD, BELFAST, BT12 6QL, TOGETHER WITH 2 PASSPORT SIZED PHOTOGRAPHS.
DVA will then complete the form, countersign it and send it to ANI. Enhanced disclosures must be signed by the Registered Body (DVA). They
must not be sent direct to ANI. One copy of the enhanced check disclosure will be sent to the DVA and one to the applicant. Please note, we have been advised by ANI that this process may take 4 weeks. Further information is available at

A criminal record will not necessarily be a bar to registration as an ADI or to being allowed to proceed with the ADI examinations. DVA as a registered body must abide by the ANI Code of Practice. This Code can be made available on request.

GUIDANCE NOTES FOR COMPLETION
In order for your application to be processed you must attach a Certificate of Good Repute from your Embassy or Consulate. You must also provide a written English translation of the certificate and documentary evidence that the translation is correct.
A digital image photograph must also accompany the application. Please see guidance notes below.
GUIDANCE FOR PASSPORT PHOTOGRAPHS
The digital image photograph must be:
- In colour, not black and white
- Taken within the last month
- Taken against a plain light grey or plain cream background
- 45 millimetres (mm) high x 35 millimetres (mm) wide
- A 300dpi resolution
- Taken with your eyes open and clearly visible (with no sunglasses or tinted glasses and no hair across your eyes)
- Free from reflection or glare on your glasses
- Free from red-eye
- Of you facing forward, looking straight at the camera
- Taken with nothing covering your face
- Be in focus and clear
- Have a strong definition between face and background
